About us
Realise is Somerset Care Group’s specialist learning disabilities service who have been recognised for the work we do, including Silver winners in the Outstanding Care Awards, and finalists in the Laing Buisson awards. We are also only one of four providers in the country to achieve national recognition for our work with people that are part of the Transforming Care agenda. Realise provides tailored support for people who may be living with their family, living in their own accommodation or to those who wish to progress to more independent living.
We are seeking Support Workers to assist in providing 3:1 support for a gentleman in his 30's living in his own home in Exeter. He is well liked and personable individual who has a slap stick sense of humour and enjoys a laugh. He has a close relationship with his family, which play an important role his life, they are involved in every aspect of his support and have a close working relationship with the team who support him.
He has multiple neurological and physiological conditions, a learning disability, severe Autism and complex communication needs. He communicates using Total Communication which includes basic sign, verbalising, gesture and body language. There are times where he finds communicating his needs and wants difficult, leading to both self injurious behaviours and behaviours that can be seen as challenging
