We are also only one of four providers in the country to achieve national recognition for our work with people that are part of the Transforming Care agenda.
Realise provides tailored support for people who may be living with their family, living in their own accommodation or to those who wish to progress to more independent living.
Positions Available:
3 x 35-hour Positive Behavioural Support Worker - £13.12 per hour (weekend uplifts of £1.00 per hour on weekends for delivered care hours)
Support individuals in their own property with all aspects of their daily life. Experience in mental health and learning disabilities is desirable.
2 x 31.5-hour per week Waking Nights Positive Behavioural Support Worker (2-week rolling rota) - £13.64 per hour (weekend uplifts of £1.00 per hour on weekends for delivered care hours)
3 nights (27 hours) one week, 4 nights (36 hours) the second week (9-hour nights).
Enable individuals who require staff support for all aspects of their night-time needs. Work collaboratively as part of a team. Experience in mental health and learning disabilities is preferable.

You’ll be working from care plans, supporting with day to day living tasks such as personal care, housekeeping, to emotional support, social interactions or accompanying those we support attend their appointments.
