Job Title: Community Care Planner

Hours: Full Time (39 hours) or Part Time

Shift:

Monday - Friday: 6.30am - 3.00pm / 9.30am - 6pm (on a rota basis)

Includes every third weekend.

Salary:

Entry: £9.05 per hour

L2 in Business Admin: £9.45 per hour

L3 in Business Admin: £9.82 per hour

Location: Wilshire Office - PB4 Ashville Centre, Commerce Way, Melksham SN12 6ZE

As a Community Care Planner with Community Wiltshire, you will be planning & organising the work of our Community-based staff in the most effective manner, whilst maintaining continuity of care for our Customers.

As part of your role, your duties will include:

  • To carry out day-to-day planning of rotas across the area.
  • To make continuous improvements to rotas to increase their effectiveness (considering travel time & time spent with customer)
  • Ensuring all schedules are complete for customers
  • To maintain accurate records & ensuring GDPR is complied with at all times.

In order to be considered for the role, you will need to:.

  • To be passionate about making a difference
  • Effective communication skills – both written and verbal
  • Excellent IT skills
  • Experience in a customer-facing role
